Information Page on the Printable Member Comment Form
Works best with Adobe Reader, Version 7
The form shown below illustrates the design and features of our Member Comment Form. The central features of the form are threefold: printability, readability, and the ability to save a copy if you wish to do so.
This form is designed to supplement SCACAI's general comment form on their website. As presently designed, the SCACAI's form 1) is a data entry form, with no print out capabilities for the user; 2) does not necessarily go to any particular member of the Board, but instead goes to a DW employee (Terry DaSilva) for distribution as she determines appropriate; and 3) is geared, according to the message on the website, to members "communicating to Association management" items of concern. As a result, it's unclear who among DW's staff will be designated to respond to your comments or whether your comments will be seen by the Board.
Print and Save Options. On the other hand, and unlike the Association's form, our form is primarily a printable form that may be utilized to craft your comments and to obtain a printout on your printer of the form for hand delivery (or for mailing) to the Administration Office, or alternatively, you may hand in the form should you decide to attend the next scheduled Board meeting. While you can still write up your comments on the spot, or use your computer's word processing program to generate a printout for the Board's response, you can now use our interactive printable form. Also, as noted in the illustration below, the form gives the reader a large type font for readability and allows you to save a copy on your computer or to make an additional copy using your printer or copier. To save, use only Adobe's "Save a Copy" command. Do not use the browser's "save as" command.
Email Option. In the case of the Board's president, our Comment Form will permit you the opportunity to email your comments directly to President Dave Weil. To do so, simply click on the "Submit by Email" button to send your comments to Mr. Weil. After a couple of self-explanatory dialogue boxes, clicking on this button will bring up your computer's email program, as explained here: Email process
Comment Form Instructions. Merely fill in the same type of information you would as if you were completing the "Blue Card" at a Board meeting. There is a built-in date calendar drop down box. In addition, you have the option of indicating the nature of the inquiry, as well as how you would like the Board to respond.
When you click on the Link "MEMBER COMMENT FORM," you will observe the following message if you do not have the latest version of Adobe Reader, Version 7. You may ignore this reminder message from Adobe by clicking the "OK" box and the Comment form will eventually open if you are patient, assuming your version of Adobe is not less than 6. We have tried and found that you should be able to print the form using Version 6.
Adobe Reader Version 7. While there may be no absolute need to upgrade to the latest version of Adobe Reader, performance in using our Comment Form would be definitely enhanced and improved if you decide to upgrade to Version 7. To do so, merely click on the "Get Adobe Reader" image below and then follow the instructions on Adobe's website.
IMPORTANT. After you are through with your comments and finished the form by either printing the form or submitting an email, and saving for form if you wish to do so, PRESS THE GREY "Reset Form" BUTTON (NOT SHOWN BELOW) LOCATED IN THE UPPER LEFT HAND CORNER TO CLEAR THE FORM OF THE INFORMATION YOU HAVE ENTERED.
